Football SA require this form to be completed by any club requesting to play games that are outside the scheduled competition fixtures, such as trial games. All games must be sanctioned by Football SA.
This form is to be completed and submitted at least 5 working days prior to the requested game. Matches requested outside of this time frame, in which a match official has been requested, may not be sanctioned.
